Procedures
- The Helen’s Wellness Clinic provides services according to the principles of normalization and person-centered planning with a positive centered approach to habilitation.
- Individuals with intellectual and developmental disabilities may be considered for placement in the adult family home if all the following criteria are met:
- The individual meets the requirements for adult family home/Helen’s Wellness Clinic level of care.
- The individual is eligible for Medicaid or will be under Helen’s Wellness clinic eligibility criteria.
- The individual meets the requirement for Helen’s Wellness Clinic level of care.
- The individual resides at Helen’s Wellness Clinic facility or at high risk of being placed in Helen’s Wellness Clinic facility
- The individual requires Helen’s Wellness Clinic services, based on medical necessity criteria, as identified through a family or person-centered planning process.
- The individual, his/her family, and /or guardian desire Helen’s Wellness Clinic participation rather than institutional services
- Persons may also be placed using private funds and as private client.
- The facility provides a family like atmosphere and provides the training necessary to help each client become as independent as possible. Each client is assisted to develop skills which allow for self-sufficiency, independence, and social acceptance in the community.
Facility rules
All Residents must Adhere to the following rules:
- Respect the rights of other clients in the facility.
- Do not smoke or use other tobacco products in the facility.
- Get ready for bed at a reasonable hour so I do not disturb others.
- Do not go into the rooms of other members of the facility.
- Do not drink alcohol beverages in the facility.
- Respect properties of others. Do not destroy their things.
